It's been out-of-print for awhile now, but I was able to procure a copy 
from a company here in Nashville.  They do a computerized search 
nationally of all booksellers who might have a spare copy lying around.  
I think my copy came to about $30.  I'll try to dig up the name and post 
back to the list.  In the meantime, I'm sure that companies who do this 
type of thing would have sites on the WWW where one could special-order 
items like this.  Does anyone have info on any?
